Doctors may also use a lumbar puncture to collect cerebrospinal fluid and measure beta-amyloid 42, tau, and phospho-tau. The National Institute on Aging's biomarker guidance emphasizes that imaging, fluid markers, and clinical findings serve different purposes and must be considered together. Blood biomarkers may help triage patients for additional testing or confirm Alzheimer's-related pathology when the test performs well. However, the Alzheimer's Association's 2025 specialty-care guideline applies these tests after objective cognitive impairment and a comprehensive clinical evaluation; many marketed tests do not meet its accuracy thresholds.
How should someone prepare for an appointment?
Useful preparation focuses on evidence the clinician needs: Genetic testing is not routinely used to diagnose or predict Alzheimer's. A neurologist or another specialist may consider it when cognitive symptoms begin early and there is a strong family history, typically with genetic counseling before or alongside testing.
