What Does the MoCA Cognitive Test Show in an Alzheimer’s Evaluation?

In an Alzheimer's evaluation, it can flag cognitive impairment, but it cannot show whether Alzheimer's disease is the cause. A low score may support the need for a fuller evaluation. What does the MoCA measure?

The MoCA is a roughly 10-minute screening test scored out of 30 points. According to MoCA Cognition's test overview, it assesses short-term memory, visuospatial and executive abilities, attention and working memory, language, and orientation. These areas matter because cognitive disorders can affect more than memory.

For example, someone may remember recent information poorly while also struggling with attention, planning, language, or awareness of time and place. The result provides a brief snapshot of cognitive performance. It does not identify the medical condition responsible for any difficulty.

What does a MoCA score mean?

MoCA Cognition describes 26 or higher as normal performance and 18–25 as mild impairment. These ranges are guides rather than diagnostic boundaries. In the original 2005 MoCA validation study indexed by PubMed, a cutoff of 26 detected 90% of participants with mild cognitive impairment and 100% with mild Alzheimer's disease.

Specificity was 87%, meaning some cognitively normal participants also scored below the cutoff. Score ranges also overlap. MoCA Cognition reports a cited average of 22 for mild cognitive impairment, with a range of 19–25, and an average of 16 for mild Alzheimer's disease, with a range of 11–21. A score in the overlap cannot determine which condition a person has.

Can the MoCA distinguish MCI from Alzheimer's dementia?

Not reliably by itself. Mild cognitive impairment (MCI) and mild Alzheimer's dementia can produce similar MoCA results. Clinically, loss of the ability to manage everyday activities helps distinguish dementia from MCI.

A clinician therefore needs to ask whether cognitive changes interfere with tasks and responsibilities, not simply whether the score falls above or below a particular number. MoCA Cognition says a score near 18 is usually used when considering the boundary between MCI and Alzheimer's dementia, but overlap occurs. A score around that point should not be treated as proof of either condition.

What else is included in an Alzheimer's evaluation?

A complete evaluation examines the person's symptoms, their daily functioning, and possible alternative explanations. The National Institute on Aging's diagnostic overview says clinicians may combine cognitive testing with physical and psychiatric assessment, laboratory testing, and, when indicated, cerebrospinal fluid testing or brain imaging. This broader process helps clinicians look for treatable alternatives as well as evidence supporting Alzheimer's.

The MoCA contributes information about current cognitive performance; it does not reveal the underlying cause. The distinction is especially important because the 2024 Alzheimer's Association workgroup criteria define Alzheimer's biologically. Under those criteria, specified abnormal biomarkers—such as validated amyloid PET, cerebrospinal fluid tests, or accurate plasma assays—can establish the disease diagnosis. A MoCA score is not one of those biomarkers.

What should happen after a concerning score?

A concerning result should lead to further assessment or a specialist referral, not an immediate Alzheimer's diagnosis. The Alzheimer's Association also says no single brief cognitive test is recognized as the best tool for deciding who requires a complete dementia evaluation. Useful questions for the clinician include: The strength of the evidence also depends on the setting.

A 2025 Alzheimer's Association draft guideline suggested against using the MoCA for early detection among U.S. English- and Spanish-speaking adults aged 55 or older in primary or ambulatory care because the accuracy evidence was rated very low. That draft recommendation does not apply to specialty memory clinics.
