How the MoCA Cognitive Test Fits Into an Alzheimer’s Diagnostic Workup

The Montreal Cognitive Assessment (MoCA) fits into an Alzheimer's workup as a brief screening test, usually near the beginning of the evaluation. It can identify cognitive problems that need further investigation, but it cannot diagnose Alzheimer's disease or determine what caused the problems. The test takes about 10 minutes and was designed to help first-line clinicians detect mild cognitive impairment (MCI). MCI means measurable cognitive decline that may be milder than dementia.

What does the MoCA measure?

The moca samples several thinking abilities: attention, executive function, memory, language, visuospatial construction, conceptual thinking, calculation, and orientation. This broad view can document which areas appear difficult during the appointment. The test produces a score out of 30.

In the original validation study, a cutoff of 26 detected 90% of participants with MCI and 100% with mild Alzheimer's, with 87% specificity among healthy controls, according to the study indexed by the U.S. National Library of Medicine. Those results demonstrate screening value, not diagnostic certainty. Scores for MCI and mild Alzheimer's can overlap, so the number does not reveal the underlying disease.

How should clinicians interpret the score?

The commonly used screening threshold is 26, but it needs context. The full MoCA adds one point for people with 12 or fewer years of education.

An abnormal score can indicate impairment without identifying whether Alzheimer's, depression, medication effects, infection, stroke, another dementia, or another condition caused it. A normal score also does not rule out impairment. The practical question is therefore not simply whether someone "passed." It is whether the result agrees with reported cognitive, behavioral, and functional changes—and whether those findings justify more evaluation.

What else belongs in the diagnostic workup?

A clinician combines cognitive testing with information from the patient and, when possible, someone who knows the patient well. The evaluation also considers changes in daily function and behavior.

What happens after a low score?

A low score should prompt additional assessment or referral rather than an immediate Alzheimer's label. The Alzheimer's Association's guidance for clinicians says brief cognitive tools identify people who need further evaluation; they cannot provide a definitive MCI or dementia diagnosis alone. Depending on the clinical picture, the next evaluation may include blood or urine tests, cerebrospinal-fluid testing, or brain imaging with CT, MRI, or PET.

These tests may support an Alzheimer's diagnosis or help identify alternatives such as stroke, tumor, infection, medication effects, depression, or another dementia. If the score is normal but cognitive or functional changes remain concerning, those concerns still warrant discussion. Normal MoCA performance does not exclude impairment.

Why the MoCA cannot determine treatment eligibility

Some Alzheimer's-directed treatments require evidence of beta-amyloid plaques. Brain imaging may also be needed to monitor amyloid-related imaging abnormalities during treatment. The National Institute on Aging's guidance for health professionals distinguishes cognitive assessment from the testing needed to support treatment decisions.

A MoCA score cannot confirm beta-amyloid plaques or provide the required imaging information. Before an appointment, bring an accurate medication list and consider asking a trusted person to describe observed cognitive, behavioral, or functional changes. Ask the clinician how education affected scoring and what additional evaluation the result supports.
