The scoring scales run in opposite directions

The 6CIT is a six-task cognitive screen scored from 0 to 28. Higher totals indicate more errors and worse cognitive performance. The Mini-Mental State Examination, or MMSE, runs from 0 to 30, with higher totals indicating better performance.

University of Bristol teaching material describes these opposing scoring directions and the 6CIT's weighted error system in its cognitive assessment guidance. Therefore, identical numbers have opposite meanings. A score of 12 represents error points on the 6CIT but points earned on the MMSE. It does not represent the same level of impairment.

The tests do not measure cognition in the same way

The 6CIT samples orientation, attention or calculation, and delayed recall through six weighted tasks. Some mistakes contribute more points than others. The MMSE is a broader screen designed to identify cognitive impairment in older adults.

The Australian Independent Health and Aged care Pricing Authority notes that it requires standardized administration and scoring guidance. Because the instruments combine different tasks and scoring rules, their totals do not sit on a common ruler. Two people can perform differently across the tested abilities even when their overall screening results appear similar.

Correlation does not create a conversion table

The tests often move in opposite directions as impairment increases: 6CIT scores rise while MMSE scores fall. That relationship explains why researchers find strong negative correlations. In a study of 253 hospital patients aged 70 or older, Tuijl and colleagues found a correlation of −0.82.

They also reported mean administration times of 2.5 minutes for the 6CIT and 5.8 minutes for the MMSE, as summarized by PubMed and the National Library of Medicine. A strong correlation means the tests tend to track related impairment across a group. It does not mean that every 6CIT total has one dependable MMSE equivalent for an individual.

Why published cutoffs do not match

Cutoffs depend on the population and the screening decision. In the hospital study, a 6CIT score of at least 11 aligned best with an MMSE score of 19 or below among lower-educated patients, but 23 or below among higher-educated patients. A separate study of 209 old-age psychiatry referrals selected thresholds of 10/11 for the 6CIT and 23/24 for the MMSE.

At the 6CIT threshold of 10/11, sensitivity was 82.5% and specificity was 90.9%; moving the threshold to 9/10 increased sensitivity to 90.2% but reduced specificity to 83.3%, according to Upadhyaya, Rajagopal and Gale's study. Sensitivity describes how often a screen identifies people who meet the study's target condition. Specificity describes how often it correctly screens out those who do not. Changing a cutoff alters that balance, so a cutoff is not a universal conversion formula.

How to interpret or report a result

Treat the score as belonging to its named instrument. Do not enter it into an unofficial conversion chart or compare it numerically with a result from the other test.

When reviewing a record or discussing a result, identify: Neither test provides a stand-alone diagnosis. If two results must be compared over time, first confirm that they came from the same instrument, version, administration method, and scoring system.
