What Causes Vascular Dementia? Risk Factors and Brain Changes Explained

Vascular dementia is caused by damage to brain blood vessels or by interrupted blood and oxygen delivery to brain tissue. How does reduced blood flow injure the brain?

Brain cells need a steady blood supply. When a clot or plaque blocks an artery, an ischemic stroke can deprive tissue of blood and cause cell death. A hemorrhagic stroke causes injury through bleeding, swelling, and increased pressure.

Vascular injury also occurs without a major recognized stroke. Chronically reduced blood flow and cerebral small-vessel disease can damage brain tissue over time. The effects depend heavily on each lesion's number, size, and location. A small injury in a strategically important area may cause substantial problems, while damage elsewhere may have less obvious effects.

What brain changes can an MRI reveal?

An MRI may show old strokes, including small "silent" strokes that did not produce an obvious recognized event. It may also reveal diseased small vessels and changes in white matter. White matter contains connections that relay messages between brain regions.

Damage can disrupt these communication routes, affecting how the brain organizes information and supports thinking or decision-making. Which conditions raise the risk?

Several conditions raise vascular-dementia risk because they damage blood vessels or increase the likelihood of impaired cerebral blood flow and stroke: These are risk factors, not proof that dementia is present. Who is affected, and does a stroke always cause dementia?

Evidence of old strokes or small-vessel disease can support a vascular contribution, but it does not automatically exclude another brain disorder.

What can reduce further brain injury?

Established dementia cannot be reversed by treating vascular risks. The practical goal is to prevent additional strokes and brain damage by identifying and treating high blood pressure, diabetes, high cholesterol, smoking, and atrial fibrillation. Evidence does not support promising that tighter blood-pressure control will prevent dementia.

In the 9,361-participant SPRINT MIND trial, intensive control reduced mild cognitive impairment but did not significantly reduce probable dementia. Early trial stopping and fewer dementia cases limited certainty, according to the NIH report on SPRINT MIND. Risk management should therefore focus on protecting blood vessels and preventing additional injury, without presenting dementia prevention as guaranteed. A practical next step is to have blood pressure, diabetes, cholesterol, smoking, and atrial fibrillation identified and appropriately treated.
