Vitamin D and Dementia Risk: Should Older Adults Be Tested?

Should Older Adults Be Tested for Vitamin D to Assess Dementia Risk?

For generally healthy older adults without symptoms or recognized risk factors, routine vitamin D screening is not supported by strong evidence. The Endocrine Society recommends against routine blood testing in healthy populations because trials have not established a vitamin D level that should be targeted to prevent specific diseases. A person who walks outdoors regularly, eats fortified foods, has no bone disease, and takes a standard-dose supplement is different from someone with osteoporosis, chronic digestive disease, or an unexplained low calcium level. Those changes can lower vitamin D levels, making deficiency partly a marker of declining health rather than its original cause. The 24-month VitaMIND trial likewise found no measurable cognitive benefit from supplementation among 620 adults aged 50 or older with mild-to-moderate deficiency and early cognitive impairment. These studies do not rule out a benefit for people with severe deficiency, but they warn against treating supplements as proven dementia prevention.

Who Has a Clinical Reason to Consider Testing?

Selective testing may be appropriate when an older adult has osteoporosis, a fragility fracture, bone pain, muscle weakness, low blood calcium, or a condition that interferes with nutrient absorption. Celiac disease, inflammatory bowel disease, pancreatic disease, and previous gastric bypass surgery are examples. Clinicians may also consider testing in people with significant kidney or liver disease or those taking medicines that affect vitamin D metabolism. Living circumstances can strengthen the case when combined with other concerns. Consider a care-home resident who rarely goes outside, eats little, has lost weight, and recently began having difficulty rising from a chair.

Interpretation is less precise than many laboratory reports imply.

According to the National Institutes of Health Office of Dietary Supplements, the National Academies considers levels below 12 ng/mL, or 30 nmol/L, associated with deficiency; levels of 20 ng/mL, or 50 nmol/L, and above are adequate for most people. Other organizations and laboratories have used different cutoffs, and the Endocrine Society has declined to set a universal target level for disease prevention. Some people need different amounts because of deficiency, malabsorption, or another medical condition. When supplementation is indicated in adults aged 50 and older, the Endocrine Society favors daily lower-dose intake over intermittent large doses. The tradeoff is that vitamin D is inexpensive and easy to take, but it is also fat-soluble and can accumulate. The adult tolerable upper intake level is 4,000 IU daily from all sources, although clinicians may temporarily prescribe more to treat documented deficiency.

Excessive intake can cause high calcium levels, nausea, weakness, confusion, kidney stones, kidney failure, abnormal heart rhythms, and other serious complications. Medication review is especially important in dementia care because several products may contain vitamin D. A person might receive a multivitamin, a calcium–vitamin D tablet, and a separate vitamin D capsule without realizing the doses overlap. Thiazide diuretics can increase the risk of high calcium when combined with vitamin D, while corticosteroids and the weight-loss medicine orlistat can interfere with vitamin D status. A pharmacist can calculate the total daily amount from prescription and over-the-counter products.

Vitamin D Within a Proper Memory Evaluation

New memory symptoms require a broader assessment than a vitamin D test. If an older adult begins missing payments, getting lost on familiar routes, repeating questions, or struggling with medications, a clinician may review daily function, perform cognitive testing, check for depression or delirium, and examine sleep, hearing, alcohol use, and medication effects. Blood and urine tests can help identify contributing conditions, but no single laboratory result establishes a dementia diagnosis.

For example, a family might attribute an abrupt week of confusion to dementia when the actual problem is an infection, dehydration, or a sedating medication. Other potentially treatable contributors include thyroid disorders and vitamin B12 deficiency. Checking vitamin D may be reasonable when risk factors are present, but ordering it alone could delay the more urgent evaluation.

Food, Sunlight, and Day-to-Day Dementia Care

Vitamin D occurs naturally in relatively few foods. Fatty fish such as salmon and trout are useful sources, while many milks, plant-based beverages, breakfast cereals, and some orange juices are fortified. Labels need to be checked because fortification varies.

A breakfast containing fortified milk and a dinner that includes salmon may provide vitamin D without adding another pill to an already complicated medication schedule. Sunlight can support vitamin D production, but it is an unreliable prescription: season, latitude, skin pigmentation, clothing, mobility, sunscreen use, and time outdoors all affect production. Deliberate unprotected exposure also carries skin-cancer risk. In a dementia-care setting, caregivers can record each supplement’s IU amount on the medication list and bring the bottles to appointments so the clinician can identify duplicate vitamin D products.
