Regular walking may be associated with a lower risk of dementia, but current evidence does not show that it prevents Alzheimer's disease. What do step-count studies show?
A UK Biobank study followed 78,430 adults for a median of 6.9 years. About 3,826 daily steps were associated with 25% lower incidence of all-cause dementia, while about 9,826 steps were associated with 51% lower incidence.
The same study linked more purposeful, faster stepping with lower dementia incidence. However, the JAMA Neurology cohort findings describe associations, not a prescribed step target or proof that walking caused the difference.
Why doesn't this prove Alzheimer's prevention?
The UK Biobank research measured all-cause dementia rather than Alzheimer's specifically. It was also observational, so researchers recorded activity and later diagnoses without assigning participants to different walking programs.
Other factors could therefore help explain the results. The study's 96.7% White sample also limits how confidently its findings can be applied to more diverse populations. These distinctions matter when interpreting a headline about "10,000 steps." The study suggests that even lower step counts may be meaningful, but it cannot promise protection at any particular number.
How important is physical activity overall?
The 2024 Lancet Commission lists physical inactivity among 14 potentially modifiable dementia risk factors. It estimated that addressing all 14 could potentially prevent or delay 45% of dementia cases across populations.
That estimate combines several risk factors. It is not a claim that walking alone prevents 45% of cases, nor does it predict what will happen to one person. This broader view fits the National Institute on Aging's assessment: physical activity may be valuable, but no clear preventive link with Alzheimer's has been documented.
What did the U.S. POINTER trial add?
The 2025 U.S. POINTER randomized trial included 2,111 at-risk adults aged 60 to 79. Participants received either structured or self-guided programs combining exercise, diet, cognitive and social activity, and vascular monitoring.
The structured program produced a statistically greater two-year improvement in global cognition. Global cognition is an overall measure drawn from cognitive testing rather than a diagnosis. The JAMA trial results cannot isolate walking's contribution because both groups received several lifestyle interventions. The study also measured cognition, not whether participants developed Alzheimer's.
A practical walking plan
Treat walking as one part of a broader activity routine, not as a stand-alone Alzheimer's prevention treatment. Avoid turning an observational finding into a rigid requirement to reach 9,826 or 10,000 steps. The World Health Organization's activity guidance gives adults 65 and older a practical framework for broad health and function:.
- Accumulate at least 150 minutes of moderate activity each week, such as brisk walking.
- Add strength work twice a week.
- Include balance-focused activity if poor mobility is a concern.
