Younger-onset families can compare blood tests by age cutoff, biomarkers measured, and counseling support. Blood biomarker counseling is a clinician-led discussion before and after testing about meaning, limits, and next steps.
The AAIC 2026 meeting took place in London on July 12-15. A Swedish study followed 1,310 patients and 165 physicians testing PrecivityAD2 in primary care. Medical Daily reported accuracy rose from 65% to 93%, matching specialists at 94%, in the Medical Daily report.

What does the primary-care finding mean for families?
A blood biomarker is a measurable blood signal tied to Alzheimer pathology. In this study, primary-care doctors used the blood result alongside usual care. That pairing lifted their accuracy close to specialist level. For families, the practical point is access.
A first discussion can start with a regular doctor rather than waiting months for specialty referral. The test remains an adjunct, not a replacement for history, exam, and clinical judgment. Early symptoms still need careful documentation. Note onset date, examples of change, medications, sleep, mood, and family history. That record helps the doctor decide whether a blood test fits.
Which blood test can you discuss before age 55?
Age eligibility now differs by product. AJMC reported FDA clearance of C2N PrecivityAD2 for symptomatic adults age 40 and older as an adjunct to workup, described in the AJMC report.
That clearance matters for younger-onset families because earlier options started at 55. Medical News Today lists three other FDA-cleared options for symptomatic adults 55 and older. They differ by biomarker target and approval date.
- Fujirebio Lumipulse p-tau217/Aβ42, cleared May 2025, measures p-tau217 relative to amyloid-beta 42.
- Roche Elecsys p-tau181, cleared Oct. 2025, measures phosphorylated tau 181.
- Roche/Eli Lilly Elecsys p-tau217, cleared Aug. 24 2026, measures p-tau217 alone.
What does a p-tau217 result signal about timing?
P-tau217 is a phosphorylated form of tau in blood that tracks Alzheimer pathology. Medscape summarized a pooled analysis of 2,684 cognitively unimpaired adults with median age 69.6 in Medscape's summary. Very high plasma p-tau217 carried 38% absolute risk of impairment at 5 years and 78% at 10 years. The same JAMA study led by Mass General Brigham found each standard-deviation rise raised progression hazard 38% with HR 1.38.
Medscape noted it added prognostic value beyond amyloid PET. That means the blood level helped forecast decline even after scan results. Columbia researchers combined p-tau217 with APOE gene testing in about 8,500 diverse participants. APOE is a gene with variants that modify Alzheimer risk. Elevated p-tau217 plus APOE4 predicted symptoms in 3-4 years, versus 5-6 years with other APOE variants.
When should counseling come before the blood draw?
Blood results do not settle the diagnosis alone. Alzheimer's Research UK stressed cleared tests are not for asymptomatic screening nor standalone diagnosis. They require history, exam, and counseling. Low results also need context. Nearly one-quarter of people with low p-tau217 still developed memory decline.
A low value lowers concern but does not remove it when symptoms persist. Early-onset disease means symptoms before 65 and accounts for about 5-10% of cases. ARUP Consult states strong family history warrants genetic counseling, with details in ARUP Consult guidance. ARUP and the Alzheimer's Association advise against APOE or biomarker testing of healthy people outside research without pre- and post-test counseling. Bring a symptom timeline, family pedigree, and medication list and ask about genetic counseling referral when symptoms appear before 65.
