CMS GUIDE 24-Hour Dementia Support in 2026: What the Update Means for Family Caregivers

Learn who qualifies for GUIDE's 24/7 line, respite funds, and how to join through a participating clinic.

CMS GUIDE still requires every participating dementia program to offer 24/7 access to help in 2026. For family caregivers, that means a live care-team contact or helpline for one-on-one support any hour. GUIDE means Guiding an Improved Dementia Experience. It is an 8-year voluntary nationwide model launched July 1, 2024 to help people with dementia stay home while easing strain on unpaid caregivers, according to the Centers for Medicare & Medicaid Services GUIDE Model page.

What the 24-hour support covers

Every participant must give each patient round-the-clock access to a care-team member or helpline. The line must take one-on-one calls from caregivers, not only patients, according to Elli Cares analysis of CMS requirements what CMS expects for 24/7 access. Each team must include a human care navigator as the main contact.

It must also include a dementia-proficient clinician able to bill Medicare Part B. Use the navigator for care plans, referrals, and follow-up. Use the 24/7 line for urgent questions, behavior changes, and nighttime crises.

Who can use GUIDE in 2026

GUIDE serves only people with clinician-diagnosed dementia who have traditional Medicare Parts A and B as primary payer. It excludes Medicare Advantage, PACE, and hospice beneficiaries.

Families must join through a participating provider. Living situation also matters this year:.

  • Residents of dedicated memory-care units are ineligible effective July 1, 2026.
  • Residents of residential-care communities need a CMS-approved partnership to take part.
  • Those residential-care residents cannot receive GUIDE respite, but keep navigation, the 24/7 line, and caregiver education.

What respite pays for now

For performance year July 1, 2026 to June 30, 2027, CMS caps GUIDE respite at $2,625 per eligible patient. It covers in-home, adult-day, or facility-based relief, according to CMS MLN Matters guidance on GUIDE payment.

Payment goes to the provider, not the family. Ask the GUIDE navigator which local respite agencies bill through the program. Schedule breaks before exhaustion sets in, not after.

How to get connected

Only about 390 sites can offer navigation, respite, and the 24/7 line. Ask your regular Part B practice if it participates in GUIDE. If not, check the CMS participant list for a nearby site and request referral.

Bring the dementia diagnosis, Medicare card, and medication list to intake. Ask for the navigator name, direct phone number, after-hours number, and respite balance. HHS made GUIDE implementation a priority in its Oct. 22, 2026 update to the national dementia plan.
