How accurate is it?
Accuracy varies by study and setting. In a population-based study of 1,119 older adults, the Mini-Cog had 76% sensitivity and 89% specificity for independently diagnosed dementia, according to Borson and colleagues. In practical terms, it missed some people with dementia and flagged some people who did not have it. The primary-care evidence is less settled.
A Cochrane review found only four eligible studies involving 1,517 participants. Sensitivity ranged from 76% to 100%, while specificity ranged from 27% to 85%. Differences among the studies and concerns about bias prevented researchers from combining the results. The review found insufficient evidence to recommend the Mini-Cog for primary-care dementia screening.
Who should interpret a score cautiously?
Clock drawing may disadvantage someone who lacks basic literacy or experience with analog clocks, even when cognition is normal. Education and cultural background therefore matter when a clinician interprets a low score. Routine screening is also a separate question from evaluating symptoms.
For adults 65 and older living in the community without symptoms, the U.S. Preventive Services Task Force found insufficient evidence to determine the overall benefits and harms of screening. The clinician may consider conditions that can resemble dementia, including depression, sleep apnea, delirium, medication effects, thyroid problems, vitamin deficiencies, and excessive alcohol use. Bring an accurate medication list and describe any observed changes, even if the Mini-Cog score appears reassuring.
