When a Mini-Cog Test Is Abnormal: What Comes Next in Dementia Testing?

What does the score mean?

The Mini-Cog uses a 0–5 scoring scale. According to the Alzheimer's Association Cognitive Assessment Toolkit, a score below 3 is validated for dementia screening. A score below 4 may justify further evaluation when greater sensitivity is desired.

The number is only one piece of evidence. Clinicians must still determine whether the person has mild cognitive impairment, dementia, or another explanation for the test result. They also identify the cognitive-behavioral syndrome—the pattern of changes in thinking, behavior, or both. That pattern helps guide the search for an underlying brain disease or contributing condition.

What will the clinician ask?

A comprehensive evaluation begins with context. The clinician gathers a history of the symptoms, their effect on daily activities, relevant risk factors, and results from additional cognitive testing. Information from a care partner can be especially useful when describing changes over time.

Examples might include difficulty managing familiar tasks, remembering plans, or following usual routines. The aim is to understand whether the problem appears on testing, in daily life, or both. The Alzheimer's Association diagnostic guideline says a comprehensive evaluation generally combines this history with cognitive assessment, laboratory testing, and brain imaging.

Could something else be contributing?

An abnormal screen can have multiple possible explanations. Primary care clinicians should investigate medication effects, mental-health conditions, infection, nutritional or metabolic abnormalities, and thyroid disease.

The workup may include: Finding a contributing condition does not automatically settle the diagnosis. A clinician may need to determine whether that condition fully explains the symptoms or exists alongside a cognitive disorder.

  • Reviewing prescription and nonprescription medicines
  • Discussing mood, behavior, and other recent health changes
  • Ordering laboratory tests for relevant medical contributors
  • Comparing cognitive findings with the person's daily functioning

Why are MRI or CT scans ordered?

Structural brain imaging—usually MRI or CT—forms one part of the evaluation. These scans can help identify alternative explanations for cognitive symptoms, including tumors, strokes, severe damage from head trauma, or excess brain fluid.

Imaging findings must be interpreted with the history, examination, cognitive testing, and laboratory results. The evaluation is designed to connect the person's functional status and symptom pattern with the most likely underlying cause.

When is specialist testing appropriate?

Specialist referral is a high priority when the diagnosis remains uncertain or symptoms are atypical or rapidly progressive. The Alzheimer's Association also identifies severe neuropsychiatric symptoms and onset before age 65 as important reasons for referral. In specialty memory care, clinicians may consider Alzheimer's blood biomarkers for people with objective cognitive impairment.

The 2025 Alzheimer's Association guideline says these tests belong within a full diagnostic workup and do not replace clinical evaluation. A sufficiently sensitive blood test may help triage suspected Alzheimer's disease. However, a positive triage result still requires confirmation with amyloid PET imaging or cerebrospinal-fluid testing, and many commercially available blood tests do not meet the guideline's accuracy thresholds.
