Tauklarify tau PET maps tau tangles in the brain, while Alzheimer's blood tests screen for amyloid-linked signals without showing spread. Families facing symptoms before age 65 should not read older-adult accuracy headlines as their own odds. Tauklarify (florquinitau F-18) is an imaging tracer for adults with cognitive impairment evaluated for Alzheimer's, approved after the Lantheus approval notice announced by Lantheus on Aug.
14, 2026. Early-onset disease starts before 65 and accounts for under 10% of cases, according to the National Institute on Aging. That small share matters because test performance in older adults does not transfer cleanly to people in their 30s, 40s, and 50s.

What each test actually shows
Tau PET visualizes the density and spread of clumped tau tangles. Amyloid PET shows a different protein. The two scans complement each other, according to SNMMI. Plasma p-tau217 and amyloid blood tests screen for amyloid-associated changes in blood.
They do not show where damage sits or how far tau has spread. That locational detail is the core difference families should keep in mind. A practical comparison is map versus smoke alarm. Tau PET offers a map of tau burden. A blood test raises an early signal that needs follow-up imaging or spinal fluid testing.
Why younger-onset headlines mislead
Most blood-test accuracy figures come from symptomatic adults age 55 and older. The first cleared test, Fujirebio Lumipulse G pTau217/β-Amyloid 1-42, was studied in 499 such patients, with 91.7% positive and 97.3% negative agreement against PET or spinal fluid, as summarized in a Frontiers review citing FDA data. Later options include Roche Elecsys pTau181 and C2N PrecivityAD2, with PrecivityAD2 indicated from age 40 for symptomatic adults. Younger adults face different causes of memory and thinking changes.
Thyroid disease, sleep apnea, depression, medication effects, frontotemporal disorders, and other neurologic conditions are relatively more common. A positive blood result is therefore less decisive without specialty workup. Genetics also narrows the story. Only a small share of younger-onset disease comes from APP, PSEN1, or PSEN2 mutations, which are nearly fully penetrant, according to Mayo Clinic and NIA genetics pages. Most early-onset cases lack those mutations and need a broader diagnostic search.
When a blood result needs backup
The Alzheimer's Association's first blood-biomarker guideline, released July 29, 2025, applies only to cognitively impaired patients seen in specialty care. According to the Association guideline summary via PR Newswire, tests need at least 90% sensitivity and 90% specificity to replace PET or spinal fluid, or at least 90% sensitivity and 75% specificity for triage use. Blood p-tau217 has everyday confounders.
Chronic kidney disease can raise it, non-fasting samples can lower it, and about 19.6% of results fall in an indeterminate range, according to Australian ACNA guidelines and JAMA data. Guidelines therefore require confirmatory PET or spinal fluid testing and prohibit asymptomatic screening or standalone diagnosis. For younger-onset families, indeterminate is not rare paperwork. It means pause, repeat under correct conditions, check kidney function, and pursue imaging or spinal fluid testing through a memory clinic.
What younger-onset families can do next
Start with specialty evaluation, not direct-access testing. Bring a timeline of symptoms, work impact, family history, medications, sleep issues, and prior labs. Ask whether blood testing would change the next step or only add noise.
Use this quick check before acting on a headline: If tau staging will affect treatment access, trial eligibility, or family planning, ask the specialist whether tau PET adds value after amyloid confirmation. Location and spread can clarify stage in ways blood cannot. Do not order tau imaging to explain a low-risk blood result alone.
- Is the person symptomatic and already referred to dementia specialty care
- Does the test match the age and indication on its label
- Is PET or spinal fluid available if the blood result is positive or unclear
- Has kidney disease, fasting status, and alternate diagnoses been addressed
