Assisted Living in New York City: Every Licensed Residence (2026)

New York City has 83 licensed adult care facilities. 17 of them hold a state Assisted Living Residence licence. Unlike nursing homes, assisted living has no federal star ratings, so we won’t pretend to rank them by quality. This page sorts them by what the state licence allows them to do for someone with dementia.

10 assisted living residences to tour first

Ranked by memory-care (SNALR) capacity, then enhanced (age-in-place) capacity. These homes can keep a resident as dementia progresses, so a parent should not have to move again a year later.

Frequently asked questions

What is the difference between ALR, EALR and SNALR in New York?

An Assisted Living Residence (ALR) provides housing, meals and help with daily activities. An Enhanced ALR (EALR) can keep a resident who needs more help, such as with walking or continence, so they can age in place. A Special Needs ALR (SNALR) is licensed to serve people with dementia, and it is the only kind of assisted living in New York that may advertise memory care.

Does New York rate assisted living facilities with stars?

No. Medicare star ratings exist only for nursing homes. For assisted living, ask the facility for its most recent NYS Department of Health inspection report and check its licence in the state directory.

Next: who pays for assisted living and memory care.

Sources and method

Nursing home ratings, staffing, inspection and fine figures are from the CMS Care Compare Provider Information dataset (CMS processing date 2026-10-01). Assisted living and memory care licences are from the New York State Department of Health Adult Care Facility Directory. Ratings change monthly, so check each facility’s current Care Compare page before you decide. No facility paid to appear here, and the reviews are written from inspection data. They are not resident testimonials.
