Race, income, and access to healthcare shape Alzheimer’s diagnosis at every stage — who develops the disease, how quickly it gets recognized, who delivers the diagnosis, and how advanced it is by the time anyone names it. Why Are Black and Hispanic Americans More Likely to Develop Alzheimer’s — and Less Likely to Be Diagnosed Early?

Rural residents face the same biological disease with fewer neurologists, fewer imaging centers, and fewer geriatric specialists within reach. A concrete example: a rural family noticing a parent’s memory decline may rely entirely on a primary care physician who sees the patient twice a year for 15 minutes.
Cognitive screening may never happen unless someone asks. If a referral is made, the nearest memory specialist might be a three-hour drive away with a months-long waitlist — a trip that requires a working car, a caregiver who can take a full day off, and a patient willing to travel while confused and anxious. Specific documentation makes it harder for symptoms to be dismissed as “normal aging” and strengthens the case for a referral or formal cognitive assessment. Medicare’s Annual Wellness Visit includes a cognitive assessment component; asking for it by name costs nothing. There is a tradeoff between waiting for a specialist and acting through primary care.
A dementia specialist evaluation is more thorough and more likely to catch the disease early — but waitlists can stretch for months. A primary care diagnosis is faster but, as the data shows, often comes later in the disease course and with less precision. A reasonable middle path: request cognitive screening and basic bloodwork through primary care immediately, while simultaneously getting on a specialist waitlist. The New Treatment Era Risks Widening the Gap
The arrival of anti-amyloid disease-modifying therapies has raised the stakes of early diagnosis considerably. These drugs are only approved for early-stage disease, which means the people diagnosed late — disproportionately Black, Hispanic, low-income, and rural patients — are often ineligible by the time their diagnosis arrives. As Practical Neurology has documented, underrepresented racial, ethnic, and socioeconomic groups face significant barriers both to early diagnosis and to accessing these new treatments, which require confirmatory amyloid testing (PET scans or lumbar punctures), regular infusions, and frequent MRI monitoring — all concentrated in well-resourced academic medical centers.
The warning here is blunt: without deliberate intervention, the treatment era could make Alzheimer’s disparities worse, not better. A therapy that only works in early-stage disease delivers its benefits almost exclusively to populations diagnosed early — which, today, means wealthier, Whiter, more urban patients. An ASPE/HHS literature review found consistent adverse disparities for Black and Hispanic populations not just in prevalence and mortality, but in clinical trial participation, medication use, long-term services, and quality of care. If the trials testing these drugs underrepresent the populations with the highest disease burden, even the evidence base itself is skewed.
Why Trust and History Still Shape Diagnosis
Diagnostic disparities are not only about money and geography — they are also about trust. Decades of documented mistreatment in medicine have left many Black and Hispanic families understandably wary of medical institutions, and that wariness can delay the first conversation about memory symptoms.
Meanwhile, standard cognitive tests were largely developed and validated on White, English-speaking, formally educated populations, which can produce misleading scores for patients with different educational or linguistic backgrounds. A practical example: a Spanish-speaking patient assessed in English with a standard screening tool may score in the “impaired” range due to language rather than cognition — or, conversely, a clinician may attribute genuine impairment to a language barrier and miss the disease. Culturally and linguistically appropriate assessment is not a courtesy; it is a diagnostic accuracy issue.
The Road Ahead — Can Blood Tests Level the Field?
There is genuine reason for cautious optimism. Blood-based biomarker tests for Alzheimer’s are moving toward routine clinical use, and they could eventually allow a primary care office in a rural county to perform the kind of biological confirmation that today requires a PET scanner in a major city. If deployed equitably — a significant “if” — cheaper, more accessible diagnostics could shrink the gap between who gets diagnosed early and who gets diagnosed in the emergency room.
But technology alone will not close disparities built on income, geography, and trust. With the Alzheimer’s population projected to reach 13.8 million Americans by 2060, the question is whether health systems will distribute new diagnostic tools toward the communities with the highest burden, or let them follow the existing channels of advantage. The historical pattern suggests the latter happens by default; the better outcome will require policy, reimbursement, and outreach designed for it.
