Doctors also assess four core clinical features: A detailed history matters because symptoms may not appear during a short appointment. A family member or caregiver can describe changes in alertness, nighttime behavior, hallucinations, movement, and everyday function that the clinician might otherwise miss.

How doctors decide whether DLB is probable
Doctors weigh the overall pattern rather than treating one symptom as proof. According to the Lewy Body Dementia Association's diagnostic assessment, DLB involves progressive cognitive decline, with attention, executive, and visuoperceptual problems often prominent early. Under the DLB Consortium criteria, probable DLB is supported by at least two core clinical features.
One core feature combined with an indicative biomarker can also support probable DLB. The American Academy of Neurology's published criteria caution that biomarkers alone should not establish probable DLB. This distinction prevents a supportive test result from outweighing the clinical picture. A reduced dopamine-transporter signal, for example, can strengthen a diagnosis when characteristic symptoms are present, but it does not replace the history and examination.
Cognitive testing and laboratory work
A clinician may begin with a brief cognitive screen. The Montreal Cognitive Assessment, or MoCA, may detect subtle LBD-related difficulties better than the Mini-Mental State Examination, or MMSE. If the screening result is unclear, a neuropsychologist can perform a more detailed assessment.
This testing maps strengths and weaknesses across attention, executive function, memory, language, and visual-spatial skills. The pattern can help the clinical team understand whether the person's difficulties resemble DLB and how those difficulties affect daily life. Blood and related laboratory tests do not prove LBD. Their purpose is to identify potentially treatable explanations or contributors, including:.

What brain scans and sleep studies can show
CT or MRI scans mainly help rule out structural causes of cognitive or movement symptoms. These include stroke, a tumor, bleeding, or hydrocephalus. A normal structural scan does not exclude DLB. A dopamine-transporter SPECT or PET scan can provide supporting evidence.
Reduced dopamine-transporter uptake in the basal ganglia, brain regions involved in movement, is consistent with DLB in the appropriate clinical setting. When REM sleep behavior disorder is suspected, a clinician may order overnight polysomnography. This sleep study can document REM sleep without normal muscle relaxation, called REM sleep without atonia. Polysomnography is the gold-standard test for confirming suspected REM sleep behavior disorder and can provide an indicative DLB biomarker.
Which specialist should evaluate suspected LBD?
A primary-care clinician is often the starting point. The National Institute on Aging's diagnosis guidance explains that diagnosis combines medical history, examinations, cognitive assessment, and tests for other causes, with no single definitive test during life. Primary care may refer the person to a cognitive or behavioral neurologist, especially when thinking changes dominate. A movement-disorder neurologist may be helpful when slowness, tremor, rigidity, or other parkinsonian features are prominent.
Other contributors may include a geriatrician, geriatric psychiatrist, or neuropsychologist. If local options are limited, ask about a dementia or movement-disorder clinic affiliated with a medical school. Before the appointment, prepare a concise timeline of cognitive, movement, sleep, hallucination, and alertness changes. Bring an up-to-date medication and substance list, and ask someone familiar with the person's daily function to attend when possible.
