Who qualifies in 2026?

What training includes

CMS materials summarized by Elli Cares say each GUIDE team must provide five core supports. They include a full assessment with a personal care plan and a dedicated navigator.

Families also get 24/7 access to a team member, caregiver classes plus support groups, and referrals for meals, transport, and community services. The CMS patient-caregiver fact sheet defines caregivers as relatives or unpaid nonrelatives who help with daily activities. They receive personal dementia training and direct navigator contact, not a salary or wage.

How need tiers work

A 2026 Journal of the American Geriatrics Society article describes the tier inputs. They are dementia stage using Clinical Dementia Rating or FAST, plus the 22-item Zarit Burden Interview for strain.

Respite and cost rules

2026 action checklist

GUIDE works only through a CMS-approved participant. BenefitGuide, citing the CMS participant spreadsheet downloaded Sept. 9, 2026, counts about 390 in Jan. 2025 and about 474 in the Aug.
