Positive predictive value (PPV) tells you the odds that a positive result reflects real disease; negative predictive value (NPV) tells you the odds that a negative result truly rules it out. A positive result means the test detected amyloid pathology in your blood. But amyloid can be present in the brain without causing symptoms or without leading to Alzheimer's disease in your lifetime. In clinical trials, about 8.3% of people who tested positive did not have confirmed pathology on brain imaging or fluid analysis, representing a false positive. Real-world accuracy may differ, especially in diverse populations where the test has not been fully validated.
If you receive a positive blood test, your doctor should order confirmatory imaging—usually an amyloid PET scan or, less commonly, a spinal fluid test via lumbar puncture. A positive blood test is a strong enough signal to justify this next step, but imaging is the current clinical standard for diagnosis. The Alzheimer's Association's 2025 guidelines recommend blood tests as a triage tool to identify who needs imaging, not as a definitive diagnosis on their own. False positives can lead to unnecessary anxiety and additional testing, which is why interpretation by a specialist familiar with both results and your symptoms matters.
False Negatives—When the Test Says No, But Symptoms Continue
A negative blood test is reassuring: 97.3% of people who test negative do not have amyloid pathology on confirmatory imaging. However, that leaves 2.7% who may have Alzheimer's pathology despite a negative blood result. This can happen if amyloid is present but at levels the test cannot yet detect, or if non-amyloid forms of dementia (such as frontotemporal dementia or Lewy body disease) are responsible for symptoms.
If your blood test is negative but cognitive symptoms persist or worsen, do not assume Alzheimer's is ruled out. Talk to your doctor about other causes, additional testing, or referral to a neurologist or memory specialist. Blood tests are one tool among many. A negative result does not end the investigation if symptoms are real and worsening.
How to Interpret Your Results and Know What Comes Next
Your blood test result should always be reviewed in the context of your age, symptoms, cognitive testing, and medical history. A single number or a "positive" or "negative" result without explanation is incomplete information. If your result is positive, expect your doctor to discuss brain imaging to confirm amyloid pathology. If your result is negative and you have no symptoms, the test has done its job: you likely do not have Alzheimer's pathology.
If your result is negative but symptoms are present, further evaluation may include neuropsychological testing, MRI to rule out other causes, or consultation with a specialist. The timing of testing also matters. Blood tests are approved for people with objective cognitive impairment—measurable decline noted on memory testing or reported by both you and someone who knows you well. Testing asymptomatic people, even with family history, is not yet recommended because we do not yet know how to manage positive results in people without symptoms.
Which Blood Tests Are Reliable, and Why Test Type Matters
Not all blood-based biomarker tests meet the same accuracy standards. Many commercial tests do not reach the performance thresholds set by the Alzheimer's Association, and validation in diverse racial and ethnic populations remains incomplete. The Lumipulse test, which measures p-tau217 and amyloid-beta 1-42, is the most rigorously validated and is the only test cleared by the FDA for this indication as of May 2025.
Research shows that combining multiple biomarkers improves accuracy. A multimarker approach using p-tau217 plus GFAP and the amyloid-beta ratio reaches an AUC of 0.97, reducing both false positives and false negatives. When you speak with your doctor about testing, ask which specific test is being used and whether it meets Alzheimer's Association guidelines. If you are being tested in a specialty memory clinic or academic medical center, you are more likely to receive a validated, high-quality test than through a direct-to-consumer service.
What Blood Tests Can and Cannot Replace
Blood tests have one major advantage over older methods: they eliminate the need for a lumbar puncture (spinal tap) to measure cerebrospinal fluid biomarkers. Plasma p-tau217 is clinically equivalent to FDA-approved CSF tests in identifying amyloid pathology, and a simple blood draw is far more acceptable to patients.
However, blood tests cannot replace brain imaging if diagnosis is needed. They also cannot replace cognitive testing, which measures whether impairment is objectively present. Blood tests are one piece of the diagnostic puzzle, not the entire picture.
