Tauklarify imaging gives primary care patients a specialist option to map tau tangles in 2026. Patients can compare this tau PET scan with blood tests and other confirmatory tests, starting with simple screening before seeking referral. Tauklarify (florquinitau F 18 injection) is a radioactive tracer for brain PET in adults with thinking problems evaluated for Alzheimer's.
PET means positron emission tomography, a scan that shows tracer activity in tissue. Lantheus Holdings reported in the FDA approval announcement that regulators cleared it on Aug. 14, 2026 to identify tau tangle pathology.

What Tauklarify Shows in the Brain
The tracer binds selectively to clumped tau protein. It makes tangles visible on PET images. It can reveal disease limited to the medial temporal lobe, the brain's small memory hub.
Doctors use it in adults with cognitive impairment evaluated for Alzheimer's. Cognitive impairment means persistent trouble with memory, thinking, or judgment. The result flags tangle pathology and does not by itself diagnose Alzheimer's.
How Accurate and Safe Is It?
Pharmacy Times summarized in its approval summary two blinded-read studies with 617 participants and five independent readers. Positive agreement ran 68%-88% and negative agreement ran 93%-99%. Readers largely agreed with each other. Limits matter for decisions. Performance was mainly validated at extremes of no versus established pathology.
A negative scan does not exclude disease, and a positive scan alone does not confirm it. Safety and effectiveness are not established for non-Alzheimer's tauopathies. In 1,734 people, headache occurred in 0.7% and nausea in 0.2%. The protocol adds radiation handling plus hydration and voiding steps. It calls for no CYP1A2 inducers, medicines that speed a liver enzyme, and no smoking for 7 days, plus a 4-hour pause in breastfeeding.
Where Do Blood Tests Fit?
Blood tests are lower-burden options that measure p-tau species, phosphorylated forms of tau in blood. BrightFocus Foundation notes in its blood-test guide four tests cleared by August 2026, including Fujirebio Lumipulse, Roche pTau181, C2N PrecivityAD2 and Roche Elecsys pTau217. The Alzheimer's Association's July 2025 guideline says blood tests with at least 90% sensitivity and 75% specificity may triage cognitively impaired specialty-care patients. Sensitivity means share of true cases found.
Positives need confirmation by spinal-fluid testing or amyloid PET. Blood draws are simpler than PET and can guide who needs further testing. Tau PET adds location detail about tangles after screening. Neither path works well as a single stand-alone answer.
What Path Can You Discuss With Your Doctor?
Start with cognitive screening and a blood biomarker in primary care. Then seek specialist referral for confirmatory amyloid PET, spinal-fluid testing, or tau PET such as Tauklarify.
Interpret each result with symptoms, history, and exam findings. Bring a current medicine list, note any smoking in the past 7 days, and mention breastfeeding because the scan protocol includes a 4-hour pause.
- Ask about memory screening and a p-tau blood test
- Review medicines, smoking history, and breastfeeding status
- Ask whether referral for amyloid PET, spinal-fluid testing, or tau PET fits
- Plan to read any single result with the full clinical picture
