Tauklarify Imaging for Alzheimer’s Tau Tangles in 2026: What Is Established and What Remains Uncertain for Younger-Onset Alzheimer’s Families

See what Tauklarify tau scans reveal, where results fall short, and what younger-onset families should ask next.

Tauklarify imaging is an approved brain scan that shows tau tangles tied to Alzheimer's disease. It gives useful added evidence for diagnosis, but one scan alone cannot prove or rule out Alzheimer's. Tauklarify (florquinitau F 18 injection) is a radioactive tracer for PET scans.

It sticks to clumps of tau protein called neurofibrillary tangles. Lantheus announced the approval in the Lantheus announcement of FDA approval on Aug. 14, 2026.

Who is the scan for?

Doctors use Tauklarify PET imaging for adults with memory or thinking problems. These adults are being checked for Alzheimer's disease. The scan looks for tau tangle pathology in the brain.

The approved use includes tangles limited to the medial temporal lobe. That small area supports memory and often shows early change. NeurologyLive describes this use as part of an Alzheimer's diagnostic workup, not as testing for healthy people.

How accurate are positive and negative results?

Approval rested on two independent studies with more than 500 participants. Five blinded readers rated each scan as positive or negative. NeurologyLive reports the study results in the NeurologyLive report on the approval with positive agreement of 68 to 88 percent and negative agreement of 93 to 99 percent.

Readers agreed strongly with each other. Reported kappa values were 0.86 to 0.92. Negative scans were more consistent than positive scans across readers.

What do results mean, and what are the limits?

A positive scan does not prove tau disease confirmed by tissue examination. A negative scan does not exclude it. Performance is lower earlier in the disease process.

The prescribing information also states a key limit. Safety and effectiveness are not established for non-Alzheimer's tau diseases. Safety testing in 1,734 people found only infrequent reactions, but labeling warns about misdiagnosis, radiation exposure, hydration, safe handling, and avoiding CYP1A2 inducers for seven days before imaging.

Does it find tangles earlier than older scans?

In a within-person comparison, MK-6240 found tangles earlier than Tauvid (flortaucipir). Being Patient reporting on the Lancet study describes about 23 extra cases found per 100 people tested in the Being Patient report on the Lancet comparison. Discrimination of Alzheimer's impairment was also higher, with AUC 0.93 versus 0.86.

Earlier detection matters because treatment choices depend on disease stage. A scan that shows limited medial temporal disease differs from one with widespread neocortical tangles. The comparison suggests Tauklarify may help place a person along that path with more precision.

Why does tau burden differ in younger-onset families?

The 2026 imaging research abstract indexed in the National Library of Medicine abstract of the 2026 younger-onset study found heavier tau burden in younger patients. Younger symptomatic patients had more severe tau buildup and lower brain metabolism than similarly impaired older patients. They also showed broader neocortical involvement and faster tau accumulation tied to thinking ability.

For families facing symptoms before age 65, that pattern can explain rapid change. A higher tangle load may match greater difficulty with planning, language, or visuospatial tasks. It does not by itself set eligibility for a drug or predict the exact course.

What should families ask the care team?

Clinicians describe tau PET as a complement to amyloid PET and other tests. Lantheus describes this role as staging and early-disease assessment. No approved Alzheimer's treatment directly targets tau, and tau PET is not a standalone diagnosis or selector for amyloid drugs.

  • Bring a list of memory changes, ages at onset, and prior amyloid or MRI results
  • Ask whether tau staging would change care, support services, or trial options
  • Ask about radiation safety, hydration, current medicines, and recent CYP1A2 inducer use
  • Ask what follow-up is planned if the scan is positive, negative, or borderline
