Tauklarify Imaging for Alzheimer’s Tau Tangles in 2026: What Changed and What Comes Next for Younger-Onset Alzheimer’s Families

See what the new tau-tangle scan adds and what steps help younger-onset families ask about access.

In August 2026, the FDA approved Tauklarify, a PET scan that lights up tau tangles in adults with cognitive impairment being evaluated for Alzheimer's. For younger-onset families, the near-term change is better early tau detection with first access through trials and specialty centers.

Tau tangles are twisted clumps of tau protein inside brain cells. They are a core marker of Alzheimer's damage. According to Lantheus in the FDA approval announcement, Tauklarify — florquinitau F 18 injection, research name MK-6240 — is used to identify that pathology during evaluation.

What does the scan show?

Tauklarify binds clumps of aggregated tau in neurofibrillary tangles. A PET camera then maps where that signal appears. The result helps show stage and spread, not just presence.

It is meant to add to amyloid PET and clinical workup, not replace them. According to Lantheus in the FDA approval announcement, the tracer complements other tools to give a fuller picture of Alzheimer's biology. Families should expect it as one part of a workup.

How is it different from the older scan?

Tauklarify is only the second FDA-approved tau PET tracer. It joins Eli Lilly's Tauvid, approved in 2020 to estimate density of aggregated tau tangles, as noted by SNMMI in its report on the approval. In the HEAD head-to-head study, 682 U.S.

and Canadian participants received both tracers. MK-6240 found tau positivity more than twice as often in early-stage regions in cognitively unimpaired people, according to results reported via AuntMinnie's coverage of The Lancet study. That edge in early regions may help catch changes sooner.

Why does earlier tau detail matter before age 65?

A diagnosis before age 65 often brings job loss, parenting stress, and misdiagnosis. Clearer biology can shorten that search. It can also guide referral to the right clinic.

Earlier onset is linked to faster tau spread that drives decline. More sensitive staging may improve trial screening and help identify low-tau patients most likely to benefit from early-stage amyloid drugs Leqembi or Kisunla. For trial candidates, timing matters.

How can families ask about access?

Wider rollout is still pending. Lantheus said it will support Alzheimer's treatment programs through its Pharma Solutions business while weighing wider commercial sale. Expect trial and specialty-center access first.

The FDA label limits use to Alzheimer's evaluation. Safety and effectiveness for other tau conditions such as frontotemporal dementia or CTE are not established. A memory center can explain whether tau PET fits the question.

  • Ask if the center offers tau PET for Alzheimer's evaluation
  • Ask if a trial uses Tauklarify for screening or staging
  • Bring symptom timeline, work impact, and family history
  • Ask what amyloid testing or exams are needed alongside it
