Before Getting a MoCA Cognitive Test for Alzheimer’s: Benefits and Limitations

Learn what may distort a MoCA score and how to decide whether a broader Alzheimer's evaluation should follow.

A MoCA cognitive test can help determine whether memory or thinking concerns warrant further evaluation, but it cannot tell you whether you have Alzheimer's disease. Its benefit is a quick, broad screen; its limitation is that the score cannot identify what caused any impairment.

The Montreal Cognitive Assessment, or MoCA, is a 30-point test that takes about 10 minutes and samples several areas of cognition. MoCA Test Inc. states that the score requires interpretation by a cognition-trained health professional as part of a complete assessment, not as a stand-alone diagnosis in its current terms of use.

What can a MoCA result tell you?

The MoCA looks for signs that someone may be having more difficulty with thinking than expected. It can give a clinician a structured starting point for discussing memory, attention, language, and other cognitive concerns. A concerning score means more evaluation may be appropriate.

It does not show that Alzheimer's caused the difficulty, establish Alzheimer's-related changes in the brain, or replace a medical assessment. A higher score is not an all-clear, either. Screening tests can miss impairment, so clinicians should also consider reported symptoms and changes in daily functioning.

How reliable is the usual cutoff?

In the original validation study, an education-adjusted score below 26 detected mild cognitive impairment with 90% sensitivity and 87% specificity. Those results came from selected clinic and control groups, however, and did not establish Alzheimer's pathology, as reported by Nasreddine and colleagues in the 2005 Journal of the American Geriatrics Society study. Sensitivity describes how often a test identifies people who have the condition being studied. Specificity describes how often it correctly classifies those who do not.

Even the original results therefore included missed cases and people incorrectly flagged. Uncertainty remains in routine care. An Alzheimer's Association 2025 public-comment draft conditionally suggested against MoCA for early detection among English- and Spanish-speaking adults age 55 and older in primary or ambulatory care because the evidence was very uncertain. This was a draft, not a final guideline, as the Association noted in its public-comment document.

Education, language, and culture affect interpretation

Education can materially influence a MoCA score. The original study found that people with 12 or fewer years of education tended to score lower, so researchers added one point to scores below 30 for that group. That adjustment does not eliminate every difference related to schooling or test familiarity.

Someone unfamiliar with the language, format, or assumptions of a task may struggle for reasons unrelated to cognitive disease. A University College London systematic review found that MoCA cutoffs and accuracy varied across cultures. Uneven study quality also limited confidence in applying one cutoff universally. Before testing, ask whether the version and interpretation are appropriate for your language, education, culture, and clinical setting.

When should testing be postponed?

A MoCA is most informative when it reflects someone's usual abilities. Temporary medical or mental states can cloud the result and make a single score harder to interpret.

Tell the clinician before testing if any of these circumstances apply: The Alzheimer's Association draft advises postponing screening or interpreting it cautiously in these situations. The practical response is usually to address the interfering condition and discuss whether testing should occur after the person has stabilized.

  • Delirium or an acute illness
  • Severe sleep deprivation
  • Sedation or a recent medication change
  • Substance withdrawal
  • A major depressive episode

What should happen after the MoCA?

A clinician should compare the result with the person's symptoms and ability to manage everyday activities. The discussion should also account for education, language, culture, testing conditions, and any temporary factors that may have affected performance. The National Institute on Aging explains that an Alzheimer's evaluation can include symptom and functional history, cognitive testing, medical and psychiatric assessment, and, when appropriate, imaging or biomarkers.

These steps help assess Alzheimer's while considering other possible causes in the NIA's diagnostic overview. After receiving the score, ask what it means in your specific circumstances, whether it matches the reported changes, and what should happen next. If the screen raises concern, request a comprehensive evaluation or an appropriate specialist referral.
