CBD and Donepezil for Dementia: Interaction and Sedation Questions to Review

Understand the evidence limits, sedation signals, and practical checks needed before combining CBD with donepezil.

Current U.S. prescribing labels do not document an interaction between cannabidiol (CBD) and donepezil, so the combination remains uncertain—not proven dangerous or proven safe.

Both can cause drowsiness or sedation, but researchers have not measured the added risk of taking them together. CBD has no FDA-approved indication for dementia. Before adding it, review the goal, dose, other medicines, alcohol use, alertness, and possible liver effects with a clinician or pharmacist.

Is a drug interaction established?

Neither the FDA's May 2026 Epidiolex label nor NIH DailyMed's July 2025 donepezil label identifies a documented CBD–donepezil interaction. That absence is reassuring only to a point. It does not prove that the combination is risk-free.

A drug-processing interaction is biologically plausible because the body metabolizes donepezil through enzymes called CYP2D6 and CYP3A4. However, prescription CBD did not change concentrations of midazolam, a sensitive CYP3A4 test drug, in a clinical study. That finding argues against assuming CBD raises every medicine processed through CYP3A4. It still does not directly answer what happens when CBD and donepezil are taken together.

How concerning is sedation?

CBD can cause somnolence, meaning unusual sleepiness, and sedation. In controlled tuberous-sclerosis-complex trials, these effects occurred in 19% of CBD recipients and 17% of placebo recipients, according to the FDA Epidiolex prescribing information. Donepezil can also contribute to drowsiness, although gastrointestinal effects and insomnia are more common. NIH DailyMed reports somnolence in 2% of donepezil recipients and under 1% of placebo recipients in mild-to-moderate Alzheimer's trials.

Those percentages should not be compared directly because they come from different patient groups and studies. CBD's clearest documented additive-sedation warning concerns alcohol and central nervous system depressants; the donepezil label does not categorize donepezil as one. If CBD is started, record any change in daytime sleepiness or alertness. Contact the prescribing clinician before increasing CBD or continuing it after a marked change.

Does CBD help dementia symptoms?

Epidiolex, the only FDA-approved CBD medicine, is approved for seizures associated with three specific disorders—not Alzheimer's disease or another dementia. CBD should therefore not be treated as an approved dementia therapy or a replacement for donepezil. A four-week randomized trial included 30 people with vascular dementia who received CBD at 300 mg daily or placebo.

CBD improved behavioral-symptom scores but produced no significant cognitive or functional benefit, according to the University of São Paulo investigators' published study. The study was small, brief, and limited to vascular dementia. It does not establish CBD's effectiveness for Alzheimer's disease or show that CBD is safe when combined with donepezil.

What should the medication review cover?

CBD can affect other medicines, making a complete review especially important for an older adult taking several drugs. In an FDA-reported 28-day study of healthy adults receiving 250 to 550 mg daily, 5.6% developed substantial elevations in ALT, a liver enzyme, versus none receiving placebo.

Bring these details to the clinician or pharmacist: Do not assume that a small dementia study or the absence of a listed interaction settles the decision. The most useful review considers the individual's full medication list, treatment goal, and ability to monitor changes in alertness.

  • The exact CBD product, planned daily amount, and reason for using it.
  • A complete list of prescriptions, nonprescription drugs, supplements, and alcohol use.
  • The current donepezil dose and timing.
  • Any existing daytime sleepiness or reduced alertness.
  • The date CBD would start, so later changes can be assessed against a clear baseline.
