– **History of gastrointestinal bleeding or active peptic ulcer disease.** Donepezil increases cholinergic activity, which can stimulate the gastrointestinal tract and potentially exacerbate ulcers or cause bleeding.
– **Asthma or chronic obstructive pulmonary disease (COPD) with active bronchospasm.** Because donepezil increases parasympathetic activity, it can cause bronchoconstriction, worsening breathing difficulties in susceptible individuals.
– **Seizure disorders.** Donepezil may lower the seizure threshold, so it should be used cautiously or avoided in patients with epilepsy or a history of seizures.
– **Urinary obstruction or severe bladder obstruction.** The drug’s cholinergic effects can increase bladder muscle tone, potentially worsening urinary retention.
– **Severe anemia.** Although less commonly emphasized, some guidelines list severe anemia as a contraindication due to the potential for exacerbating hypoxia or other systemic effects.
– **Concurrent use with certain medications that may interact adversely.** For example, combining donepezil with other cholinergic drugs or drugs that affect heart rhythm may increase risks.
In addition to these absolute contraindications, donepezil must be used cautiously in patients with:
– **Cardiac conditions such as sick sinus syndrome or other conduction defects.** Even if not severe, these conditions require close monitoring.
– **Gastrointestinal conditions prone to irritation or bleeding.**
– **Patients with a history of seizures or epilepsy.**

Donepezil’s mechanism of increasing acetylcholine levels in the nervous system underlies many of these contraindications because acetylcholine affects multiple organ systems, including the heart, lungs, gastrointestinal tract, and urinary system. Therefore, any condition that could be worsened by increased parasympathetic (cholinergic) activity needs to be carefully considered before starting donepezil.
In clinical practice, doctors evaluate these contraindications along with the patient’s overall health status and other medications to decide whether donepezil is appropriate.
